Our history

  • AGAPE Community Health Center, Inc. is a Florida 501 (c) 3, not-for-profit,  organization established in 2004 as a co-applicant with the Duval County Health Department, to provide quality healthcare services for the insured, uninsured, and underinsured residents of Jacksonville, FL.
  • On April 1, 2016, AGAPE Community Health Center, Inc. was independently awarded the designation as the only 330 (e) – FQHC in Jacksonville, FL.

  • A community mainstay, AGAPE operates six (6) facilities, which  include  four (4)  Comprehensive  Primary  Care Centers, two (2) Women’s Health Center (AGAPE – Magnolia East & Merrill) and one (1) Community Partnership School (Ed White High School) which is located in an underserved community.

  • During COVID-19 pandemic response, AGAPE maintained two (2) COVID Sites that were open to the entire city where we provided education, testing and vaccinations.
  • AGAPE is governed by a remarkably dedicated board of directors (more than 51% of the members are users of the medical services) and we have an incredibly talented, diverse, and committed family of providers and staff. AGAPE’s multicultural, multilingual, and multidisciplinary staff services a diverse population of 18,000 patients annually.

Since 2004, Agape Family Health has been serving Jacksonville as a Non-profit Health Center providing a variety of health services with a focus on holistic care.

We accept most medical health insurance including Medicaid and Medicare and also offers a sliding fee discount program for those who are uninsured.

Agape offers comprehensive, integrated care including Primary Medical Care, Beharioral Health, Vision and Pharmaceutical Services.

The Problem

Access to comprehensive, quality health care services is important for promoting and maintaining health, preventing and managing disease, reducing unnecessary disability and premature death, and achieving a high health equity.

However, a large majority of our Jacksonville community face barriers to accessing health services including: inadequate or lack of insurance, limited or lack of transportation to where health services are provided, disparities in care linked to economic, sexual orientation, gender, race/ethnicity, disability status, etc., and limited or lack of access to proper nutrition and health education guidance. Delays in receiving appropriate care and unmet medical needs cause higher rates of preventable conditions, increased hospitalizations, and high mortality rates.

Agape Family Health is passionate about ensuring exceptional health care services are accessible to the entirety of our Jacksonville community- no matter who they are, where they are, and whenever they need it.

We achieve this goal by providing easier and greater access to those that are not only insured, but also to those who are underserved and uninsured. Agape Family Health’s providers are advocates for their patients- and strive to build a relationship through trust, communication, and education.

Our organization leaders focus on on facilitating mobile medical services to those in need and developing strategic partnerships along with collaborations to create and fund sustainable programs that will further our vision of accessible care for all.
