AHF is the only section 330(e) Federally qualified health center in Jacksonville.  Agape provides comprehensive medical, pharmaceutical and behavioral health services.

Primary Care

Choosing the right Primary Care provider for you and your family is an important decision.  Agape Family Health (AFH) has Board-certified Physicians and Nurse Practitioners who are experienced and well-qualified to treat your illness.  Our providers are available when you need them.


Women's Health

AFH encourages women to have examinations, including screening for cervical cancer, clinical breast exams and mammograms. We offer a variety of services for women, including well woman's care and family planning. Expectant mothers can receive prenatal care, counseling, nutritional education, and other services at AFH.



AHF has board-certified Pediatricians and Nurse Practitioners that will give your child the love and care they deserve.  Many of our providers are also bilingual to help make communicating more effective.   We offer sick and well child visits, immunizations, hearing and vision screenings, school and sports physicals.


Behavioral Health

 Our highly qualified Behavioral Health Team is available to assist patients needing more intense or customized treatment plans, including the treatment of PTSD, Anxiety and Depression, Bipolar Disorder, ADHD, Obesity, Marital Distress, Chronic Tension, Panic Attacks, as well as Phobias.


diabetes education Classes

Twenty-nine million Americans have diabetes.  Another 86 million Americans, one in three adults, have prediabetes,  AFH has a care team trained to help control diabetes.    Our nurse team will help you to obtain a dietary plan, set goals and develop creative, effective ways for lowering your blood sugar levels.



Prescription Medication is also available at AFH.


DOT Physical Exams

Agape offers Department of Transportation Physical exams.  This exam consists of an evaluation of vision, hearing, blood pressure and pulse rate, along with overall physical ability.  It also tests for illegal substance.     Please bring a complete list of any current medications.  Call to schedule your exam today.

Agape Family Health Mobile Unit

Access to Quality Healthcare in the Community

Agape Family Health (AFH) Mobile Medical Unit is AFH medical unit on wheels providing access to quality health care for men, women and children ages 3 and up with a focus on eliminating barriers to care for underserved children and the importance of reducing health care disparities. 

The AFH Medical Mobile unit is staffed by a team of experienced health care professionals, with support of community resources for Medical Mobile Unit patients.

Mobile Unit Community Goals:


  • Educate families of health insurance options
  • Educate families on the value and advantages of establishing a permanent medical home for their children

  • Promote health education and healthy living in the communities served 

All Patients are Welcome

We welcome all patients, regardless of ability to pay. If you do not have health insurance, we will work with you to determine your discounted affordable payment. 

No appointment is necessary. Patients will be seen in the order of arrival.
