Our Vision

AGAPE will be recognized in our community as the premiere primary care practice that offers patient-centered, affordable healthcare for all people. We will be an organization that attracts high-quality healthcare professionals who strive to improve the quality of life for the entire community. We will continue to competitively grow to meet the healthcare needs of our patients, staff and the community.

History

  • AGAPE Community Health Center, Inc. is a Florida 501 (c) 3, not-for- profit,  organization established in 2004 as a co-applicant with the Duval County Health Department, to provide quality healthcare services for the insured, uninsured, and under insured residents of Jacksonville, FL.

  • On April 1, 2016, AGAPE Community Health Center, Inc. was independently awarded the designation as the only 330 (e) - FQHC in Jacksonville, FL.

  • A community mainstay, AGAPE operates six (6) facilities, which  include  four (4)  Comprehensive  Primary  Care Centers, two (2) Women’s Health Center (AGAPE – Magnolia East & Merrill) and one (1) Community Partnership School (Ed White High School) which is located in an underserved community.

  • During COVID-19 pandemic response, AGAPE maintained two (2) COVID Sites that were open to the entire city where we provided education, testing and vaccinations.

  • AGAPE is governed by a remarkably dedicated board of directors (more than 51% of the members are users of the medical services) and we have an, incredibly talented, diverse, and committed family of providers and staff. AGAPE’s multicultural, multilingual, and multidisciplinary staff services a diverse population of 18,000 patients annually.
